Your First Appointment

To help you prepare for your first visit to our office, please bring:

• Your eyeglasses and sunglasses
• Your contact lenses or boxes from disposables as well as your solutions
• List any medications you take routinely as well as any eye medications you use
• Records you have of previous eye examinations and test results
• Your insurance cards and appropriate referral forms

Depending on the nature of your visit and your medical conditions, you may need to have someone drive you to our office. The doctor may give you eye drops which could make your eyes sensitive to light and cause your vision to be slightly out of focus for several hours.

For your convenience we accept Visa, MasterCard and ATM cards.

Cancellation Policy

 Your time is valued at our office. Each patient is scheduled an appropriate block of time. When someone misses a scheduled appointment or is late it affects all other patients scheduled that day. Therefore, we reserve the right to reschedule anyone who arrives more than five minutes late for their scheduled appointment.

If you need to change or cancel a scheduled appointment we require a twenty four hour notice.

Failure to do so will result in a $25 charge.
